Add hibernation support to xhci platform by adding
the proper PM ops freeze and restore.
Also set the hibernated argument to xhci_resume which
will trigger the reset-resume of the connected
root-hubs and the devices.

Pixel 2 field testers reported that when they tried to reboot their
phones with some USB devices plugged in, the reboot would get wedged and
eventually trigger watchdog reset. Once the Pixel kernel team found a
reliable repro case, they narrowed it down to this commit's 4.4.y
backport. Reverting the change made the issue go away.

The flag skip_resume is set when DWC3 is already runtime
suspended and prepare it for PM suspend. But if PM suspend of
DWC is not called because of some reason, then directly the PM
resume of xhci-plat is called which invokes xhci_resume when DWC
is in Low Power Mode. This leads to watchdog timeout.
Fix this issue by not calling xhci_resume if skip_resume is set.

situations where kexec might be used by stopping DMA
transactions.

DWC3 driver uses wakeup_source and allows pm_suspend only after
entering low power mode as part of runtime suspend. This prevents
PM suspend in host mode if connected device's driver doesn't
support runtime or selective suspend.
Add support in driver where user can specify using dtsi attribute
to not use wakeup source in host mode. It will allow system to enter
deep sleep or pm_suspend irrespective of runtime PM state of XHCI.
On high level below are the changes in default behavior:
-Do not use wakeup_source for DWC3 when operating in host mode.
-For host mode, devices will suspend upon PM suspend and resumed
after PM resume.

platform_get_irq() returns an error code, but the xhci-plat driver
ignores it and always returns -ENODEV. This is not correct, and
prevents -EPROBE_DEFER from being propagated properly.

Latency involved in handling dwc3 interrupt in real time threaded
interrupt is sometimes resulting into RT throttling. Hence queue
bottom half work from dwc3 hard irq handler to a high priority
workqueue. Also, update the corresponding irq flag in host
controller platform driver.

Upstream commit 98d74f9ceaef ("xhci: fix 10 second timeout on removal of
PCI hotpluggable xhci controllers") fixes a problem with hot pluggable PCI
xhci controllers which can result in excessive timeouts, to the point where
the system reports a deadlock.
The same problem is seen with hot pluggable xhci controllers using the
xhci-plat driver, such as the driver used for Type-C ports on rk3399.
Similar to hot-pluggable PCI controllers, the driver for this chip
removes the xhci controller from the system when the Type-C cable is
disconnected.
The solution for PCI devices works just as well for non-PCI devices
and avoids the problem.

xhci quirks is initialized in xhci_gen_setup() which gets called
from usb_add_hcd(). Since LPM support quirk is ORed to xhci quirk
before calling usb_add_hcd(), quirk gets overwritten. Fix this issue
by moving LPM support quirk to xhci_plat_quirks() which gets called
after xhci quirk is initialized.

The commit 4ac53087d6 ("usb: xhci: plat: Create both
HCDs before adding them") move add hcd to the end of
probe, this cause hcc_params uninitiated, because xHCI
driver sets hcc_params in xhci_gen_setup() called from
usb_add_hcd().
This patch checks the Maximum Primary Stream Array Size
in the hcc_params register after add primary hcd.

XHCI allows interrupt moderation using imod_interval at 250ns
increments. Add DT parameter to specify this imod_value for
targets mainly with single CPU to reduce CPU interrupt loads.
This allows better balance between CPU usage and performance.

As part of bus suspend if xHC is halted when there are pending
events in the event ring, xHC stops responding to xhci commands
upon host or device initiated bus resume. Fix this issue by
going over all the pending events and writing address of xHC
event ring enqueue pointer to ERDP register. This indicates to
xHC that system software has processed all Events in the ring.

This allows xHC to remain in low power mode and not
resume the bus when system wide resume happens.
Controller comes out of low power mode only when usb
device issues a remote wakeup or if there is a host
initiated bus resume.

xHC gets powered down when host mode is stopped. xHCI power
management is intended for ACPI based power management which
is defined for PC architecture. Hence there is no need to
clear run/stop bit and save the runtime register states upon
port suspend.

During system suspend, usb_suspend() will get called for
both root hub instances and in the choose_wakeup() routine
it will find that device_may_wakeup() returns false. If
the busses were previously runtime suspended then
udev->do_remote_wakeup would be set, but since it conflicts
with the wakeup setting being false, the routine has to
first runtime resume the devices in order to change the
remote wakeup setting. This causes unnecessary delay
as it interrupts the system suspend sequence.
Since the host already does support remote wakeup, enable the
per-device wakeup settings for both the hcd root hub instances
in order to avoid the mismatch during choose_wakeup() which
will allow usb_suspend() to proceed without any unnecessary
resume.

Commit 5dcd26f4e903 ("xhci: fix 10 second timeout on removal of PCI
hotpluggable xhci controllers") avoids queueing configure_endpoint
commands for the dropped endpoints.
This change adds XHCI_STATE_REMOVING flag on removal of usb device.
For usb XHCI hotplugged controllers this will prevent 5 second command
timeouts and avoid the races with mutex lock.

On some platforms, the clocks might be registered by a platform
driver. When this is the case, the clock platform driver may very well
be probed after xhci-plat, in which case the first probe() invocation
of xhci-plat will receive -EPROBE_DEFER as the return value of
devm_clk_get().
The current code handles that as a normal error, and simply assumes
that this means that the system doesn't have a clock for the XHCI
controller, and continues probing without calling
clk_prepare_enable(). Unfortunately, this doesn't work on systems
where the XHCI controller does have a clock, but that clock is
provided by another platform driver. In order to fix this situation,
we handle the -EPROBE_DEFER error condition specially, and abort the
XHCI controller probe(). It will be retried later automatically, the
clock will be available, devm_clk_get() will succeed, and the probe()
will continue with the clock prepared and enabled as expected.
In practice, such issue is seen on the ARM64 Marvell 7K/8K platform,
where the clocks are registered by a platform driver.

There is a possibility of dwc3 irq being fired as soon as
it gets enabled by tasklet handler. As a result dwc3 hard irq
handler disables the irq and schedules tasklet. If tasklet is
still running, tasklet_schedule becomes no op and dwc3 irq
remains disabled permanently. Fix this issue by handling irq
in threaded context with IRQF_ONESHOT flag set. Also update
the shared irq flag in xhci platform driver for irq registration
in HCD driver.

xhci allows interrupts to be moderated at 250ns interval;
provide an sysfs interface to modify interrupt rate from
userspace to understand throughput variation by changing imod.
Usage:
Below command will moderate interrupts at 1ms or (4000 * 250ns)
interval.
echo 4000 > /sys/devices/8a00000.ssusb/8a00000.dwc3/xhci-hcd.0.auto/

Provide the methods to let ACPI identify the need to use
xhci-platform. Change the Kconfig files so the
xhci-plat.o file is selectable during kernel config.
This has been tested on an ARM64 machine with platform XHCI, an
x86_64 machine with XHCI, and an x86_64 machine without XHCI.
There were no regressions or error messages on the machines
without platform XHCI.

The xhci platform driver needs to work on systems that
either only support 64-bit DMA or only support 32-bit DMA.
Attempt to set a coherent dma mask for 64-bit DMA, and
attempt again with 32-bit DMA if that fails.

In the OTG case, the controller might not yet have been
added or is removed before the system suspends.
Assign xhci->main_hcd during probe to prevent NULL
pointer de-reference in xhci_suspend/resume().
Use the hcd->state flag to check if HCD is halted
and if that is so do nothing for xhci_suspend/resume().

As xhci_hcd is now allocated by usb_create_hcd(), we don't
need to add the primary HCD before creating the shared HCD.
Creating the shared HCD before adding the primary HCD is particularly
useful for the OTG use case so that we know at the OTG core if
the HCD is in single configuration or dual (primary + shared)
configuration.

HCD core allocates memory for HCD private data in
usb_create_[shared_]hcd() so make use of that
mechanism to allocate the struct xhci_hcd.
Introduce struct xhci_driver_overrides to provide
the size of HCD private data and hc_driver operation
overrides. As of now we only need to override the
reset and start methods.

The commit 9737479285 ("usb: host: xhci-plat: add support for the Armada
375/38x XHCI controllers") extended the xhci-plat driver to support the Armada
375/38x SoCs, mostly by adding a quirk configuring the MBUS window.
However, that quirk was run before the clock the controllers needs has been
enabled. This usually worked because the clock was first enabled by the
bootloader, and left as such until the driver is probe, where it tries to
access the MBUS configuration registers before enabling the clock.
Things get messy when EPROBE_DEFER is involved during the probe, since as part
of its error path, the driver will rightfully disable the clock. When the
driver will be reprobed, it will retry to access the MBUS registers, but this
time with the clock disabled, which hangs forever.
Fix this by running the quirks after the clock has been enabled by the driver.

When system is being suspended, if host device is not allowed to do wakeup,
xhci_suspend() needs to clear all root port wake on bits. Otherwise, some
platforms may generate spurious wakeup, even if PCI PME# is disabled.
The initial commit ff8cbf250b ("xhci: clear root port wake on bits"),
which also got into stable, turned out to not work correctly and had to
be reverted, and is now rewritten.

Instead of building all of the xHCI code into a single module, separate
it out into the core (xhci-hcd), PCI (xhci-pci, now selected by the new
config option CONFIG_USB_XHCI_PCI), and platform (xhci-plat) drivers.
Also update the PCI/platform drivers with module descriptions/licenses
and have them register their respective drivers in their initcalls.

Since the struct hc_driver is mostly the same across the xhci-pci,
xhci-plat, and the upcoming xhci-tegra driver, introduce the function
xhci_init_driver() which will populate the hc_driver with the default
xHCI operations. The caller must supply a setup function which will
be used as the hc_driver's reset callback.
Note that xhci-plat also overrides the default ->start() callback so
that it can do rcar-specific initialization.

The R-Car H2 and M2 SoCs come with an xHCI controller that requires
some specific initializations related to the firmware downloading and
some specific registers. This patch adds the support for this special
configuration as an xHCI quirk executed during probe and start.

Some platforms (such as the Renesas R-Car) need to initialize some specific
registers after xhci driver calls usb_add_hcd() and before the driver calls
xhci_run(). So, this patch adds the xhci_plat_start() function.

If we build a kernel with PM_SUSPEND set and no PM_SLEEP,
we get a build warning in the xhci-plat driver about unused
functions.
To fix this, use "#ifdef CONFIG_PM_SLEEP", like we do in most
other drivers nowadays.

The Armada 375 and 38x SoCs come with an XHCI controller that requires
some specific initialization related to the MBus windows
configuration. This patch adds the support for this special
configuration as an XHCI quirk executed during probe.
Two new compatible strings are added to identify the Armada 375 and
Armada 38x XHCI controllers, and therefore enable the relevant quirk.

Some platforms (such as the Armada 38x ones) can gate the clock of
their USB controller. This patch adds the support for one clock in
xhci-plat, by enabling it during probe and disabling it on remove.
To achieve this, it adds a 'struct clk *' member in xhci_hcd. While
only used for now in xhci-plat, it might be used by other drivers in
the future. Moreover, the xhci_hcd structure already holds other
members such as msix_count and msix_entries, which are MSI-X specific,
and therefore only used by xhci-pci.
