The DOMAIN_ATTR_COHERENT_HTW_DISABLE IOMMU domain attribute is being
removed. SMMU coherency will be configured through the SMMU device tree
nodes moving forward. Remove the obsolete option.

Some SMMUs can walk page tables in the CPU cache. Enable this
behavior for SMMUs whose device tree node has the `coherent'
property set.

The disable_bypass cmdline option changes the SMMUv3 driver to put down
faulting stream table entries by default, as opposed to bypassing
transactions from unconfigured devices.
In this mode of operation, it is entirely expected to see aborting
entries in the stream table if and when we come to installing a valid
translation, so don't trigger a BUG() as a result of misdiagnosing these
entries as stream table corruption.

In the unlikely event of a global command queue error, the ARM SMMUv3
driver attempts to convert the problematic command into a CMD_SYNC and
resume the command queue. Unfortunately, this code is pretty badly
broken:
1. It uses the index into the error string table as the CMDQ index,
so we probably read the wrong entry out of the queue
2. The arguments to queue_write are the wrong way round, so we end up
writing from the queue onto the stack.
These happily cancel out, so the kernel is likely to stay alive, but
the command queue will probably fault again when we resume.
This patch fixes the error handling code to use the correct queue index
and write back the CMD_SYNC to the faulting entry.

Due to the limitations of having to wait until we see a device's DMA
restrictions before we know how we want an IOVA domain initialised,
there is a window for error if a DMA ops domain is allocated but later
freed without ever being used. In that case, init_iova_domain() was
never called, so calling put_iova_domain() from iommu_put_dma_cookie()
ends up trying to take an uninitialised lock and crashing.
Make things robust by skipping the call unless the IOVA domain actually
has been initialised, as we probably should have done from the start.

The default domain for a device might also be
identity-mapped. In this case the kernel would crash when
unity mappings are defined for the device. Fix that by
making sure the domain is a dma_ops domain.

Removal of IOMMU driver cannot be done reliably, so Exynos IOMMU driver
doesn't support this operation. It is essential for system operation, so
it makes sense to prevent unbinding by disabling bind/unbind sysfs
feature for SYSMMU controller driver to avoid kernel ops or trashing
memory caused by such operation.

Ensure that on detach any lazy mappings for that device are released.
We don't want to leave any outstanding mappings after detach because
the client expects all mappings to have been unmapped, also we don't
want to leave references, in the lazy mapping framework, to devices
which could cease to exists.

Consider any pre-initialized stream match register table entries,
which are of type translation, as static stream ID to context bank
mappings.
When an attach occurs for a stream ID which has a static context bank
mapping ensure that the pre-initialized stream match register table
entry is used for that stream ID.
Contract for using static context banks:
1) Before doing the attach hardware translations must be stopped and
hardware translations are only re-enabled once the required mappings
have been added.
2) Before the attach the static CB must be in stage 1 context bank
bypass (CBn_SCTLR.M == 0).

In general, the IOMMU framework likes to group PCI devices together.
This prevents someone from assigning them to separate VMs using VFIO,
for example. The reason they shouldn't go to separate VMs is because
there could be DMA aliasing between endpoint devices, and because the
root complex device can access anything that endpoint devices can
access.
Unfortunately, this makes things difficult in the DMA layer since the
IOMMU framework freaks out if you try to attach a single device that is
also part of a group. First instinct might be to start using
iommu_attach_group instead of iommu_attach_device in the DMA layer but
that has problems of its own (it attaches *everyone* in the group as
soon as one client attaches). Besides all that, it's also a bit
annoying to have the root complex device in group since, in our
platform, it doesn't actually have any stream IDs assigned to it and it
doesn't do any DMA of its own, so it doesn't really make any logical
sense to attach it.
As a compromise, we can stop grouping PCI devices together by allocating
a new domain for each one. By doing so we no longer need to call
iommu_attach_group in the DMA layer, avoid the issues described above.
It should be noted that with this patch in place we can't securely do
any direct DMA assignment of PCI endpoint devices to separate VMs.

There is a race condition in the AMD IOMMU init code that
causes requested unity mappings to be blocked by the IOMMU
for a short period of time. This results on boot failures
and IO_PAGE_FAULTs on some machines.
Fix this by making sure the unity mappings are installed
before all other DMA is blocked.

This seems to be required on some X58 chipsets on systems
with more than one IOMMU. QI does not work until it is
enabled on all IOMMUs in the system.

Currently iommu-debug was enable only for 64 bit
targets as there were compilation issues coming
up while enabling it for 32 bit. Fix these issues
to ensure that we can use it on 32 bit targets
too.

context banks. Increase readabilty by printing a human-readable device
name, instead of just the streamid.

TLB invalidation is done during unmap with clocks already enabled, but
we still have calls to enable/disable clocks in
arm_smmu_tlb_inv_range_nosync. Remove the extra calls.

A common software error when it comes to page table code is missing TLB
maintenance. Add some checks to the io-pgtable-fast code to detect when
an address that might be stale in the TLB is being re-used. This can be
accomplished by writing a "stale TLB" flag value to the reserved bits of
the PTE during unmap and then removing the flag value when the TLBs are
invalidated (by sweeping the entire page table). That way, whenever we
map we can know that there might be a stale TLB in the location being
mapped into if it contains the "stale TLB" flag value.

It's fairly common while debugging to need to enable the config clocks
for an SMMU so that you can poke around at the registers. Add a debugfs
file to do this.

The vanilla ARM DMA IOMMU mapper is used by many clients in our system,
but we have no functional test coverage of it. Add some functional
testing for it by leveraging the tests that were recently added for the
Fast DMA mapper. Since the Fast mapper and the ARM mapper are both DMA
API implementations we can share most of the code.

io-pgtable-fast does some underhanded tricks to achieve performance.
One of those tricks is that it expects clients to call its map function
directly, rather than going through the IOMMU framework. Add a DMA API
implementation that goes through io-pgtable-fast.

The upcoming "fast" DMA mapper will need to take control of TLB
invalidation. Doing so allows us to perform fewer TLB invalidation
operations since the DMA mapper layer has more knowledge about when
"stale" TLB entries might actually become a problem, so it can do TLB
invalidation much less frequently. Implement the tlbi_domain op for
this purpose.

Certain use cases require performance that can't be achieved with the
general-purpose SMMU page table code. By limiting ourselves to 4K page
mappings (no block mappings) and pre-populating the first and second
levels of the page tables up front, we can eliminate a lot of the work
needed for page table mapping and unmapping.
Add a performance-tuned io-pgtable implementation for ARMv8L page tables
that only supports 4K page mappings. Any size can be mapped, but only
4K page mappings will be installed in the page tables.

Currently we restrict the pgsize_bitmap for the entire SMMU every time
we allocate some new page tables. However, certain io-pgtable
implementations might wish to restrict the formats beyond the
restrictions of the SMMU itself, which forces all domains on that SMMU
to the same pgsize_bitmap, even if the other domains would prefer to use
a more permissive page table format. Besides that, some SMMUs in the
system might have different supported page sizes at the hardware level,
so applying those to everyone else is wrong.
Fix these issues by implementing the new .get_pgsize_bitmap IOMMU op.

Currently we use a single pgsize_bitmap per IOMMU driver. However, some
IOMMU drivers might service different IOMMUs with different supported
page sizes. Some drivers might also want to restrict page sizes for
different use cases. Support these use cases by adding a
.get_pgsize_bitmap function to the iommu_ops which can optionally be
used by the driver to return a domain-specific pgsize_bitmap.
