gadget_driver can become null if gadget_stop is called while any other
gadget callbacks are in progress. As gadget callbacks needs to release
spinlock before the callback, store the local copy of gadget_driver to
avoid the race with gadget_stop.

There are 2 control endpoint structures for DWC3. However, the driver
only updates the OUT direction control endpoint structure during
ConnectDone event. DWC3 driver needs to update the endpoint max packet
size for control IN endpoint as well. If the max packet size is not
properly set, then the driver will incorrectly calculate the data
transfer size and fail to send ZLP for HS/FS 3-stage control read
transfer.
The fix is simply to update the max packet size for the ep0 IN direction
during ConnectDone event.

According to the databook ep0 should be in setup
phase during reset. If host issues reset between
control transfers, ep0 will be in an invalid state.
Fix this my issuing stall and restart on ep0 if it
is not in setup phase.

So far, dwc3 has always missed request->zero
handling for every endpoint. Let's implement
that so we can handle cases where transfer must
be finished with a ZLP.
Note that dwc3 is a little special. Even though
we're dealing with a ZLP, we still need a buffer
of wMaxPacketSize bytes; to hide that detail from
every gadget driver, we have a preallocated buffer
of 1024 bytes (biggest bulk size) to use (and
share) among all endpoints.

In the case of bounced ep0 requests, we must delay DMA operation until
after ->complete() otherwise we might overwrite contents of req->buf.
This caused problems with RNDIS gadget.

Latency involved in handling dwc3 interrupt in real time threaded
interrupt is sometimes resulting into RT throttling. Hence queue
bottom half work from dwc3 hard irq handler to a high priority
workqueue. Also, update the corresponding irq flag in host
controller platform driver.

Implement interrupt moderation which allows the interrupt rate to be
throttled. To enable this feature the dwc->imod_interval must be set to
1 or greater. This value specifies the minimum inter-interrupt interval,
in 250 ns increments. A value of 0 disables interrupt moderation.
This applies for DWC_usb3 version 3.00a and higher and for DWC_usb31
version 1.20a and higher.

Now that all the infrastructure is in place, we can
clear events in the top-half handler in order to
bring IRQ line low ASAP.
This is also a necessary step in order to implement
workaround for known erratum in follow-up patches.

Endpoint command to request resources on ep0 is take upto ~800us and
this delay is observed for every new/re enumeration. As the api polls
(reads the register and it happens over cnoc) for every 1us to check
for completion it could result in congestion on cnoc. Hence increase
polling interval from 1us to 20us for this endpoint command.

When device controller gets an LPM request from the host during a
control transfer, it is not able to initiate remote wakeup
automatically. As a result it accepts the request, goes to L1 state,
and does not initiate wakeup to exit L1 state. Since host expects
the device to do a remote wakeup from L1 state and the device does
not initiate the wakeup, the host continues to wait and starts
enumeration again because control transfer timeout occurs. Fix this
issue by initiating remote wakeup before queuing the ep0 request
if bus is in L1 suspend state. Also add a counter which gets
incremented upon device sending remote wakeup before queuing ep0
request.

commit 0416e494ce ("usb: dwc3: ep0: correct cache
sync issue in case of ep0_bounced") introduced a bug
where we would leak DMA resources which would cause
us to starve the system of them resulting in failing
DMA transfers.
Fix the bug by making sure that we always unmap EP0
requests since those are *always* mapped.

Format specifier %p can leak kernel addresses while not valuing the
kptr_restrict system settings. When kptr_restrict is set to (1), kernel
pointers printed using the %pK format specifier will be replaced with 0's.
Debugging Note : &pK prints only Zeros as address. If you need actual
address information, write 0 to kptr_restrict.

Current code is resizing each USB IN endpoint's TXFIFO to 1KB if it is
not USB GSI endpoint or bursting is not enable. In some high throughput
test case where there is contention in terms of accessing DDR memory
through SNOC between USB3 and other peripheral PCIE, there are many
underrun errors (i.e. Bad Data Length/Bad CRC) observed. These errors
results into retransmision of USB packets and sometime dropping packets
causing diffrent kind of data stalls. Hence fix this issue by increasing
TXFIFO size as 3KB with endless USB IN endpoint (i.e. USB BAM2BAM
endpoint) which helps to successfully retransmit packets.

For DBM endpoints, queueing endless request through generic ep_queue
API should not allowed. Otherwise it might lead to crash when called
dwc3_gadget_map_request(). Hence add check whether it is DBM endpoint
or not and if it is, return error.

Upon host bus reset driver draws 0mA and switches
to higher value 900/500 mA after moving to configured
state. This causes a glitch in the led and battery icon
status. Hence request to draw 100mA to void the glitch.

Controller driver explicitly puts the controller in low power mode
when cable disconnected. However, due to config-fs and ADB design
which unbinds composition on every cable disconnect results in
detaching/stopping gadget driver on every cable disconnect. Gadget
stop will explicitly brings the controller out of low power mode to
disable events and ep0 which is not necessary as controller is
already in low power mode. These operations are not required for
composition switch as well because gadget pull-up call back will
take care of that. Hence, Remove disabling events and ep0 from stop
gadget callback.

In case of usb bus suspend at the time of disconnect, rmnet function
suspend disables the endpoint. Same endpoint gets disabled again as
result of handling gadget driver disconnect. This generates warning
message and dumps the call stack. This may cause message flooding
as a result of multiple usb plug in/plug out. Hence replace
dev_WARN_ONCE with dev_dbg and log this event in debug buffer.

USB GSI endpoint related TRB ring is allocated with EP_OPS. Each USB
endpoint related TRB ring is zeroed with usb_ep_disable() operation. Hence
if USB composition switch or USB cable disconnect is performed when there
is active data transfer with USB GSI endpoint, it results into IPA GSI
accessing 0x0 address causing bus errors. Hence fix this issue by not
memsetting TRB ring with zero for USB GSI endpoint.

XFERNOTREADY event provides status about current transfer. Hence
if transfer is active, then avoid starting next transfer by calling
__dwc3_gadget_kick_transfer() API. Otherwise USB controller stalls to
perform data transfer on that endpoint due to starting new transfer
without completing in progress transfer.

("usb: dwc3: gadget: use update transfer command"). If we get
a Xfer Not Ready event with reason "Transfer Active" issuing
update transfer for the endpoint for any pending request
results into transfer stall on the endpoint. Hence do not use
update transfer command for on-demad transfers.

This change reverts Commit f3af36511e ("usb: dwc3: gadget: always
enable IOC on bulk/interrupt transfers") which sets IOC bit for both
bulk/interrupt transfers resulting into interrupt for each TRB
completion with event as XFERINPROGRESS. In some cases it has been
observed that controller is not able to complete provided TRB on
bulk-in endpoint causing stall conditions. Hence Fix this issue by not
setting IOC bit for each TRB to receive XFERCOMPLETE interrupt with it.

commit 0b93a4c838 ("usb: dwc3: fix TRB completion when multiple TRBs
are started") enables XFERINPROGRESS event with all endpoint type
except control endpoint. Currently we are not using XFERINPROGRESS for
queuing next request and depends on XFERNOTREADY event only.

Let's fix that up by incrementing request->actual
only once.

Sometime endpoint command timeout happening after
usb device enumeration. Controller stops responding to
any endpoint commands afterwards. To recover from this
condition restart usb by simulating vbus off and on
except for end transfer command. Also increase the
command timeout from 1500us to 3000us.

commit f3af36511e ("usb: dwc3: gadget: always
enable IOC on bulk/interrupt transfers") ended up
regressing Isochronous endpoints by clearing
DWC3_EP_BUSY flag too early, which resulted in
choppy audio playback over USB.
Fix that by partially reverting original commit and
making sure that we check for isochronous endpoints.

There is a possibility of tasklet bottom half handler racing
with dwc3_msm_suspend(). As a result before disabling the dwc3
irq, dwc3 interrupts are fired and once the suspend routine
disables the clocks bottom half handler gets a chance to run
and tries to access dwc3 register resulting into un-clocked
register access. Since dwc3 interrupt is already running in
threaded irq context, interrupt can be directly handled in
same context and avoid this race.

In RNDIS composition when windows PC is suspended RNDIS
driver sends flow control enable which frees the trb pool
of the RNDIS endpoints and trb pool pointer is set to NULL.
When bus suspend happens RNDIS gsi driver performs endpoint
disable operation because remote wake up is disabled. Endpoint
disable perform memset 0 on trb pool which is already set to
NULL causing the NULL pointer dereference. Fix this by adding
NULL check for trb pool before doing memset 0.
