Impact: fix possible race condition in ftrace function return tracer
This fixes a possible race condition if index incrementation
is not immediately flushed in memory.

Impact: fix crash when enabling the branch-tracer
When the branch tracer inserts an event through
probe_likely_condition(), it calls local_irq_save() and then results
in a trace recursion.
local_irq_save() -> trace_hardirqs_off() -> trace_hardirqs_off_caller()
-> unlikely()
The trace_branch.c file is protected by DISABLE_BRANCH_PROFILING but
that doesn't prevent from external call to functions that use
unlikely().
My box crashed each time I tried to set this tracer (sudden and hard
reboot).

Impact: extend the ->init() method with the ability to fail
This bring a way to know if the initialization of a tracer successed.
A tracer must return 0 on success and a traditional error (ie:
-ENOMEM) if it fails.
If a tracer fails to init, it is free to print a detailed warn. The
tracing api will not and switch to a new tracer will just return the
error from the init callback.
Note: this will be used for the return tracer.

Impact: set filtered functions at time the filter is set
It can be confusing when the set_filter_functions is set (or cleared)
and the functions being recorded by the dynamic tracer does not
match.
This patch causes the code to be updated if the function tracer is
enabled and the filter is changed.

Impact: correct implementation of dyn ftrace filter
The old decisions made by the filter algorithm was complex and incorrect.
This lead to inconsistent enabling or disabling of functions when
the filter was used.
This patch simplifies that code and in doing so, corrects the usage
of the filters.

Impact: make ftrace_convert_nops() more permissive
Due to the way different architecture linkers combine the data sections
of the mcount_loc (the section that lists all the locations that
call mcount), there may be zeros added in that section. This is usually
due to strange alignments that the linker performs, that pads in zeros.
This patch makes the conversion code to nops skip any pointer in
the mcount_loc section that is NULL.

Impact: allow archs more flexibility on dynamic ftrace implementations
Dynamic ftrace has largly been developed on x86. Since x86 does not
have the same limitations as other architectures, the ftrace interaction
between the generic code and the architecture specific code was not
flexible enough to handle some of the issues that other architectures
have.
Most notably, module trampolines. Due to the limited branch distance
that archs make in calling kernel core code from modules, the module
load code must create a trampoline to jump to what will make the
larger jump into core kernel code.
The problem arises when this happens to a call to mcount. Ftrace checks
all code before modifying it and makes sure the current code is what
it expects. Right now, there is not enough information to handle modifying
module trampolines.
This patch changes the API between generic dynamic ftrace code and
the arch dependent code. There is now two functions for modifying code:
ftrace_make_nop(mod, rec, addr) - convert the code at rec->ip into
a nop, where the original text is calling addr. (mod is the
module struct if called by module init)
ftrace_make_caller(rec, addr) - convert the code rec->ip that should
be a nop into a caller to addr.
The record "rec" now has a new field called "arch" where the architecture
can add any special attributes to each call site record.

Impact: fix lockdep disabling itself when function tracing is enabled
The raw_local_irq_saves used in ftrace is causing problems with
lockdep. (it thinks the irq flags are out of sync and disables
itself with a warning)
The raw ops here are not needed, and the normal local_irq_save is fine.

Impact: keep from converting freed records
When the tracer is started or stopped, it converts all code pointed
to by the saved records into callers to ftrace or nops. When modules
are unloaded, their records are freed, but they still exist within
the record pages.
This patch changes the code to skip over freed records.

Impact: robust feature to disable ftrace on start or stop tracing on error
Currently only the initial conversion to nops will disable ftrace
on an anomaly. But if an anomaly happens on start or stopping of the
tracer, it will silently fail.
This patch adds a check there too, to disable ftrace and warn if the
conversion fails.

Impact: let module functions be recorded when dyn ftrace not enabled
When dynamic ftrace had a daemon and a hash to record the locations
of mcount callers at run time, the recording needed to stop when
ftrace was disabled. But now that the recording is done at compile time
and the ftrace_record_ip is only called at boot up and when a module
is loaded, we no longer need to check if ftrace_enabled is set.
In fact, this breaks module load if it is not set because we skip
over module functions.

Inotify watch removals suck violently.
To kick the watch out we need (in this order) inode->inotify_mutex and
ih->mutex. That's fine if we have a hold on inode; however, for all
other cases we need to make damn sure we don't race with umount. We can
*NOT* just grab a reference to a watch - inotify_unmount_inodes() will
happily sail past it and we'll end with reference to inode potentially
outliving its superblock.
Ideally we just want to grab an active reference to superblock if we
can; that will make sure we won't go into inotify_umount_inodes() until
we are done. Cleanup is just deactivate_super().
However, that leaves a messy case - what if we *are* racing with
umount() and active references to superblock can't be acquired anymore?
We can bump ->s_count, grab ->s_umount, which will almost certainly wait
until the superblock is shut down and the watch in question is pining
for fjords. That's fine, but there is a problem - we might have hit the
window between ->s_active getting to 0 / ->s_count - below S_BIAS (i.e.
the moment when superblock is past the point of no return and is heading
for shutdown) and the moment when deactivate_super() acquires
->s_umount.
We could just do drop_super() yield() and retry, but that's rather
antisocial and this stuff is luser-triggerable. OTOH, having grabbed
->s_umount and having found that we'd got there first (i.e. that
->s_root is non-NULL) we know that we won't race with
inotify_umount_inodes().
So we could grab a reference to watch and do the rest as above, just
with drop_super() instead of deactivate_super(), right? Wrong. We had
to drop ih->mutex before we could grab ->s_umount. So the watch
could've been gone already.
That still can be dealt with - we need to save watch->wd, do idr_find()
and compare its result with our pointer. If they match, we either have
the damn thing still alive or we'd lost not one but two races at once,
the watch had been killed and a new one got created with the same ->wd
at the same address. That couldn't have happened in inotify_destroy(),
but inotify_rm_wd() could run into that. Still, "new one got created"
is not a problem - we have every right to kill it or leave it alone,
whatever's more convenient.
So we can use idr_find(...) == watch && watch->inode->i_sb == sb as
"grab it and kill it" check. If it's been our original watch, we are
fine, if it's a newcomer - nevermind, just pretend that we'd won the
race and kill the fscker anyway; we are safe since we know that its
superblock won't be going away.
And yes, this is far beyond mere "not very pretty"; so's the entire
concept of inotify to start with.

A common reason for device drivers to implement their own printk macros
is the lack of a printk prefix with the standard pr_xyz macros.
Introduce a pr_fmt() macro that is applied for every pr_xyz macro to the
format string.
The most common use of the pr_fmt macro would be to add the name of the
device driver to all pr_xyz messages in a source file.

This reverts commit e51af66308, which was
wrongly hoovered up and submitted about a month after a better fix had
already been merged.
The better fix is commit cbda1ba898
("PCI/iommu: blacklist DMAR on Intel G31/G33 chipsets"), where we do
this blacklisting based on the DMI identification for the offending
motherboard, since sometimes this chipset (or at least a chipset with
the same PCI ID) apparently _does_ actually have an IOMMU.

We don't want to get rid of the futexes just at exit() time, we want to
drop them when doing an execve() too, since that gets rid of the
previous VM image too.
Doing it at mm_release() time means that we automatically always do it
when we disassociate a VM map from the task.

dpt_i2o.c::adpt_i2o_to_scsi() reads the value at (reply+5) which
should contain the length in bytes of the transferred data. This
would be correct if reply was a u32 *. However it is a void * here,
so we need to read the value at (reply+20) instead.
The value at (reply+5) is usually 0xff0000, which is apparently
'large enough' and didn't cause any trouble until 2.6.27 where
commit 427e59f09f

We do this by abandoning the global list of SMB sessions and instead
moving to a per-server list. This entails adding a new list head to the
TCP_Server_Info struct. The refcounting for the cifsSesInfo is moved to
a non-atomic variable. We have to protect it by a lock anyway, so there's
no benefit to making it an atomic. The list and refcount are protected
by the global cifs_tcp_ses_lock.
The patch also adds a new routines to find and put SMB sessions and
that properly take and put references under the lock.

Currently libata uses four methods to detect device presence.
1. PHY status if available.
2. TF register R/W test (only promotes presence, never demotes)
3. device signature after reset
4. IDENTIFY failure detection in SFF state machine
Combination of the above works well in most cases but recently there
have been a few reports where a phantom device causes unnecessary
delay during probe. In both cases, PHY status wasn't available. In
one case, it passed #2 and #3 and failed IDENTIFY with ATA_ERR which
didn't qualify as #4. The other failed #2 but as it passed #3 and #4,
it still caused failure.
In both cases, phantom device reported diagnostic failure, so these
cases can be safely worked around by considering any !ATA_DRQ IDENTIFY
failure as NODEV_HINT if diagnostic failure is set.

The code that allows these structs to be shared is extremely racy.
Disable the sharing of SMB and tcon structs for now until we can
come up with a way to do this that's race free.
We want to continue to share TCP sessions, however since they are
required for multiuser mounts. For that, implement a new (hopefully
race-free) scheme. Add a new global list of TCP sessions, and take
care to get a reference to it whenever we're dealing with one.

Commit 46abc02175 ("phylib: give mdio
buses a device tree presence") added a call to device_unregister() in
a situation where the caller did not intend for the device to be
freed yet, but apart from just unregistering the device from the
system, device_unregister() does an additional put_device() that is
intended to free it.
The right function to use in this situation is device_del(), which
unregisters the device from the system like device_unregister() does,
but without dropping the reference count an additional time.

This is the next page of the scm recursion story (the commit
f8d570a4 net: Fix recursive descent in __scm_destroy()).
In function scm_fp_dup(), the INIT_LIST_HEAD(&fpl->list) of newly
created fpl is done *before* the subsequent memcpy from the old
structure and thus the freshly initialized list is overwritten.
But that's OK, since this initialization is not required at all,
since the fpl->list is list_add-ed at the destruction time in any
case (and is unused in other code), so I propose to drop both
initializations, rather than moving it after the memcpy.
Please, correct me if I miss something significant.
