cdbf92675f mm: numa: avoid waiting on freed migrated pages

In do_huge_pmd_numa_page(), we attempt to handle a migrating thp pmd by
waiting until the pmd is unlocked before we return and retry.  However,
we can race with migrate_misplaced_transhuge_page():

This can result in the freed page having its waiters flag set
unexpectedly, which trips the PAGE_FLAGS_CHECK_AT_PREP checks in the
page alloc/free functions.  This has been observed on arm64 KVM guests.

We can avoid this by having do_huge_pmd_numa_page() take a reference on
the page before dropping the pmd lock, mirroring what we do in
__migration_entry_wait().

When we hit the race, migrate_misplaced_transhuge_page() will see the
reference and abort the migration, as it may do today in other cases.

7128b46468 mm: avoid taking zone lock in pagetypeinfo_showmixed()
pagetypeinfo_showmixedcount_print is found to take a lot of time to
complete and it does this holding the zone lock and disabling interrupts.
In some cases it is found to take more than a second (On a 2.4GHz,8Gb
RAM,arm64 cpu).  Avoid taking the zone lock similar to what is done by
read_page_owner, which means possibility of inaccurate results.

1f2284fac2 mm: fix new crash in unmapped_area_topdown()

Trinity gets kernel BUG at mm/mmap.c:1963! in about 3 minutes of
mmap testing.  That's the VM_BUG_ON(gap_end < gap_start) at the
end of unmapped_area_topdown().  Linus points out how MAP_FIXED
(which does not have to respect our stack guard gap intentions)
could result in gap_end below gap_start there.  Fix that, and
the similar case in its alternative, unmapped_area().

4b35943067 mm: larger stack guard gap, between vmas

Stack guard page is a useful feature to reduce a risk of stack smashing
into a different mapping. We have been using a single page gap which
is sufficient to prevent having stack adjacent to a different mapping.
But this seems to be insufficient in the light of the stack usage in
userspace. E.g. glibc uses as large as 64kB alloca() in many commonly
used functions. Others use constructs liks gid_t buffer[NGROUPS_MAX]
which is 256kB or stack strings with MAX_ARG_STRLEN.

This will become especially dangerous for suid binaries and the default
no limit for the stack size limit because those applications can be
tricked to consume a large portion of the stack and a single glibc call
could jump over the guard page. These attacks are not theoretical,
unfortunatelly.

Make those attacks less probable by increasing the stack guard gap
to 1MB (on systems with 4k pages; but make it depend on the page size
because systems with larger base pages might cap stack allocations in
the PAGE_SIZE units) which should cover larger alloca() and VLA stack
allocations. It is obviously not a full fix because the problem is
somehow inherent, but it should reduce attack space a lot.

One could argue that the gap size should be configurable from userspace,
but that can be done later when somebody finds that the new 1MB is wrong
for some special case applications.  For now, add a kernel command line
option (stack_guard_gap) to specify the stack gap size (in page units).

Implementation wise, first delete all the old code for stack guard page:
because although we could get away with accounting one extra page in a
stack vma, accounting a larger gap can break userspace - case in point,
a program run with "ulimit -S -v 20000" failed when the 1MB gap was
counted for RLIMIT_AS; similar problems could come with RLIMIT_MLOCK
and strict non-overcommit mode.

Instead of keeping gap inside the stack vma, maintain the stack guard
gap as a gap between vmas: using vm_start_gap() in place of vm_start
(or vm_end_gap() in place of vm_end if VM_GROWSUP) in just those few
places which need to respect the gap - mainly arch_get_unmapped_area(),

I saw need_resched() warnings when swapping on large swapfile (TBs)
because continuously allocating many pages in swap_cgroup_prepare() took
too long.

We already cond_resched when freeing page in swap_cgroup_swapoff().  Do
the same for the page allocation.

bfbd244c5f mm/memory-failure.c: use compound_head() flags for huge pages
commit 7258ae5c5a2ce2f5969e8b18b881be40ab55433d upstream.

memory_failure() chooses a recovery action function based on the page
flags.  For huge pages it uses the tail page flags which don't have
anything interesting set, resulting in:

> Memory failure: 0x9be3b4: Unknown page state
> Memory failure: 0x9be3b4: recovery action for unknown page: Failed

Instead, save a copy of the head page's flags if this is a huge page,
this means if there are no relevant flags for this tail page, we use the
head pages flags instead.  This results in the me_huge_page() recovery
action being called:

> Memory failure: 0x9b7969: recovery action for huge page: Delayed

For hugepages that have not yet been allocated, this allows the hugepage
to be dequeued.

bcfa0a9140 mm: turn vmap_purge_lock into a mutex
The purge_lock spinlock causes high latencies with non RT kernel.  This
has been reported multiple times on lkml [1] [2] and affects
applications like audio.

This patch replaces it with a mutex to allow preemption while holding
the lock.

Thanks to Joel Fernandes for the detailed report and analysis as well as
an earlier attempt at fixing this issue.

0818bc7a05 mm: refactor __purge_vmap_area_lazy()
Move the purge_lock synchronization to the callers, move the call to
purge_fragmented_blocks_allcpus at the beginning of the function to the
callers that need it, move the force_flush behavior to the caller that
needs it, and pass start and end by value instead of by reference.

No change in behavior.

28a6e877ad mm/vmalloc: keep a separate lazy-free list
When mixing lots of vmallocs and set_memory_*() (which calls
vm_unmap_aliases()) I encountered situations where the performance
degraded severely due to the walking of the entire vmap_area list each
invocation.

One simple improvement is to add the lazily freed vmap_area to a
separate lockless free list, such that we then avoid having to walk the
full list on each purge.

the reason is that the managed memory is too low (only 110MB) while the
rest of the the 50GB is still waiting for the deferred intialization to
be done.  update_defer_init estimates the initial memoty to initialize
to 2GB at least but it doesn't consider any memory allocated in that
range.  In this particular case we've had

	Reserving 4096MB of memory at 128MB for crashkernel (System RAM: 51200MB)

so the low 2GB is mostly depleted.

Fix this by considering memblock allocations in the initial static
initialization estimation.  Move the max_initialise to
reset_deferred_meminit and implement a simple memblock_reserved_memory
helper which iterates all reserved blocks and sums the size of all that
start below the given address.  The cumulative size is than added on top
of the initial estimation.  This is still not ideal because
reset_deferred_meminit doesn't consider holes and so reservation might
be above the initial estimation whihch we ignore but let's make the
logic simpler until we really need to handle more complicated cases.

In __munlock_pagevec() we should decrement NR_MLOCK for each page where
we clear the PageMlocked flag.  Commit 1ebb7cc6a5 ("mm: munlock: batch
NR_MLOCK zone state updates") has introduced a bug where we don't
decrement NR_MLOCK for pages where we clear the flag, but fail to
isolate them from the lru list (e.g.  when the pages are on some other
cpu's percpu pagevec).  Since PageMlocked stays cleared, the NR_MLOCK
accounting gets permanently disrupted by this.

Fix it by counting the number of page whose PageMlock flag is cleared.

necessary update to the hugepage ref-count.

But when !hugepage_migration_supported() , unmap_and_move_hugepage()

Address this by changing the putback_active_hugepage() in
soft_offline_huge_page() to putback_movable_pages().

This only triggers on systems that enable memory failure handling

I imagine this wasn't triggered as there aren't many systems running

14bfe118dd slub/memcg: cure the brainless abuse of sysfs attributes

memcg_propagate_slab_attrs() abuses the sysfs attribute file functions
to propagate settings from the root kmem_cache to a newly created
kmem_cache.  It does that with:

     attr->show(root, buf);
     attr->store(new, buf, strlen(bug);

Aside of being a lazy and absurd hackery this is broken because it does
not check the return value of the show() function.

Some of the show() functions return 0 w/o touching the buffer.  That
means in such a case the store function is called with the stale content
of the previous show().  That causes nonsense like invoking
kmem_cache_shrink() on a newly created kmem_cache.  In the worst case it
would cause handing in an uninitialized buffer.

This should be rewritten proper by adding a propagate() callback to
those slub_attributes which must be propagated and avoid that insane
conversion to and from ASCII, but that's too large for a hot fix.

Check at least the return value of the show() function, so calling
store() with stale content is prevented.

8b26f53bf0 mm/huge_memory.c: respect FOLL_FORCE/FOLL_COW for thp
commit 8310d48b125d19fcd9521d83b8293e63eb1646aa upstream.

In commit 19be0eaffa3a ("mm: remove gup_flags FOLL_WRITE games from
__get_user_pages()"), the mm code was changed from unsetting FOLL_WRITE
after a COW was resolved to setting the (newly introduced) FOLL_COW
instead.  Simultaneously, the check in gup.c was updated to still allow
writes with FOLL_FORCE set if FOLL_COW had also been set.

However, a similar check in huge_memory.c was forgotten.  As a result,
remote memory writes to ro regions of memory backed by transparent huge
pages cause an infinite loop in the kernel (handle_mm_fault sets
FOLL_COW and returns 0 causing a retry, but follow_trans_huge_pmd bails
out immidiately because `(flags & FOLL_WRITE) && !pmd_write(*pmd)` is
true.

While in this state the process is stil SIGKILLable, but little else
works (e.g.  no ptrace attach, no other signals).  This is easily
reproduced with the following code (assuming thp are set to always):

Fix this by updating follow_trans_huge_pmd in huge_memory.c analogously
to the update in gup.c in the original commit.  The same pattern exists
in follow_devmap_pmd.  However, we should not be able to reach that
check with FOLL_COW set, so add WARN_ONCE to make sure we notice if we
ever do.

929dac4900 kasan: add functions to clear stack poison
Functions which the compiler has instrumented for ASAN place poison on
the stack shadow upon entry and remove this poison prior to returning.

In some cases (e.g. hotplug and idle), CPUs may exit the kernel a
number of levels deep in C code.  If there are any instrumented
functions on this critical path, these will leave portions of the idle
thread stack shadow poisoned.

If a CPU returns to the kernel via a different path (e.g. a cold
entry), then depending on stack frame layout subsequent calls to
instrumented functions may use regions of the stack with stale poison,
resulting in (spurious) KASAN splats to the console.

Contemporary GCCs always add stack shadow poisoning when ASAN is
enabled, even when asked to not instrument a function [1], so we can't
simply annotate functions on the critical path to avoid poisoning.

Instead, this series explicitly removes any stale poison before it can
be hit.  In the common hotplug case we clear the entire stack shadow in
common code, before a CPU is brought online.

On architectures which perform a cold return as part of cpu idle may
retain an architecture-specific amount of stack contents.  To retain the
poison for this retained context, the arch code must call the core KASAN
code, passing a "watermark" stack pointer value beyond which shadow will
be cleared.  Architectures which don't perform a cold return as part of
idle do not need any additional code.

This patch (of 3):

The commit '6b4f77 (mm: vmscan: invoke slab shrinkers from shrink_zone())'
fixed the invocation of shrinkers but resulted in lowmemorykiller shrinker
being called more. Reduce the number of lowmemorykiller shrinker
invocations by separating out the lowmemorykiller shrinker from
shrink_slab. This will make lowmemorykiller invoked only once for all zones
reclaimed in the direct reclaim path, and once for each zone in the kswap
path. As a consequence the eligible pages passed to shrink_slab_lmk is now
the reclaimable pages of all zones. Reducing the number of lowmemorykiller
invocations reduces the unnecessary time spent in lowmemorykiller and thus
contention or failures on lowmemorykiller's scan_mutex.

49118fe6a3 mm: enable page poisoning early at boot
On SPARSEMEM systems page poisoning is enabled after buddy is up, because
of the dependency on page extension init.  This causes the pages released
by free_all_bootmem not to be poisoned.  This either delays or misses the
identification of some issues because the pages have to undergo another
cycle of alloc-free-alloc for any corruption to be detected.

Enable page poisoning early by getting rid of the PAGE_EXT_DEBUG_POISON
flag.  Since all the free pages will now be poisoned, the flag need not be
verified before checking the poison during an alloc.

with __GFP_ZERO can be skipped as well.  The tradeoff is that detecting
corruption from the poisoning is harder to detect.  This feature also
cannot be used with hibernation since pages are not guaranteed to be
zeroed after hibernation.

5c69adad61 mm/page_poison.c: enable PAGE_POISONING as a separate option
Page poisoning is currently set up as a feature if architectures don't
have architecture debug page_alloc to allow unmapping of pages.  It has
uses apart from that though.  Clearing of the pages on free provides an
increase in security as it helps to limit the risk of information leaks.
Allow page poisoning to be enabled as a separate option independent of
kernel_map pages since the two features do separate work.  Because of
how hiberanation is implemented, the checks on alloc cannot occur if
hibernation is enabled.  The runtime alloc checks can also be enabled
with an option when !HIBERNATION.
