commit 9608e5c0f079390473b484ef92334dfd3431bb89 upstream.
This patch adds a device ID for the RT Systems cable used to
program Yaesu VX-8R/VX-8DR handheld radios. It uses the main
FTDI VID instead of the common RT Systems VID.

commit d2471d4a24dfbff5e463d382e2c6fec7d7e25a09 upstream.
In the earlier commit dad3f793f20f ("usb: dwc2: Make sure we
disconnect the gadget state"), I was trying to fix up the
fact that we somehow weren't disconnecting the gadget state,
so that when the OTG port was plugged in the second time we
would get warnings about the state tracking being wrong.
(This seems to be due to a quirk of the HiKey board where
we do not ever get any otg interrupts, particularly the session
end detected signal. Instead we only see status change
interrupt.)
The fix there was somewhat simple, as it just made sure to
call dwc2_hsotg_disconnect() before we connected things up
in OTG mode, ensuring the state handling didn't throw errors.
But in looking at a different issue I was seeing with UDC
state handling, I realized that it would be much better
to call dwc2_hsotg_disconnect when we get the state change
signal moving to host mode.
Thus, this patch removes the earlier disconnect call I added
and moves it (and the needed locking) to the host mode
transition.

disabled by USB host and free the whole list of completed out

to user memory.
To prevent from this let's remove our working element from

finish with it.

[ Upstream commit dad3f793f20fbb5c0c342f0f5a0bdf69a4d76089 ]
I had seen some odd behavior with HiKey's usb-gadget interface
that I finally seemed to have chased down. Basically every other
time I plugged in the OTG port, the gadget interface would
properly initialize. The other times, I'd get a big WARN_ON
in dwc2_hsotg_init_fifo() about the fifo_map not being clear.
Ends up if we don't disconnect the gadget state, the fifo-map
doesn't get cleared properly, which causes WARN_ON messages and
also results in the device not properly being setup as a gadget
every other time the OTG port is connected.
So this patch adds a call to dwc2_hsotg_disconnect() in the
reset path so the state is properly cleared.
With it, the gadget interface initializes properly on every
plug in.

commit 1a087f032111a88e826877449dfb93ceb22b78b9 upstream.
When I debug a kernel crash issue in funcitonfs, found ffs_data.ref
overflowed, While functionfs is unmounting, ffs_data is put twice.
Commit 43938613c6fd ("drivers, usb: convert ffs_data.ref from atomic_t to
refcount_t") can avoid refcount overflow, but that is risk some situations.
So no need put ffs data in ffs_fs_kill_sb, already put in ffs_data_closed.
The issue can be reproduced in Mediatek mt6763 SoC, ffs for ADB device.
KASAN enabled configuration reports use-after-free errro.

commit cb88a0588717ba6c756cb5972d75766b273a6817 upstream.
Corsair Strafe RGB keyboard does not respond to usb control messages
sometimes and hence generates timeouts.
Commit de3af5bf259d ("usb: quirks: add delay init quirk for Corsair
Strafe RGB keyboard") tried to fix those timeouts by adding
USB_QUIRK_DELAY_INIT.
Unfortunately, even with this quirk timeouts of usb_control_msg()
can still be seen, but with a lower frequency (approx. 1 out of 15):
[ 29.103520] usb 1-8: string descriptor 0 read error: -110
[ 34.363097] usb 1-8: can't set config #1, error -110
Adding further delays to different locations where usb control
messages are issued just moves the timeouts to other locations,
e.g.:
[ 35.400533] usbhid 1-8:1.0: can't add hid device: -110
[ 35.401014] usbhid: probe of 1-8:1.0 failed with error -110
The only way to reliably avoid those issues is having a pause after
each usb control message. In approx. 200 boot cycles no more timeouts
were seen.
Addionaly, keep USB_QUIRK_DELAY_INIT as it turned out to be necessary
to have the delay in hub_port_connect() after hub_port_init().
The overall boot time seems not to be influenced by these additional
delays, even on fast machines and lightweight distributions.

commit 17aa31f13cad25daa19d3f923323f552e87bc874 upstream.
This fixes an issue that a gadget driver (usb_f_fs) is possible to
stop rx transactions after the usb-dmac is used because the following
functions missed to set/check the "running" flag.
- usbhsf_dma_prepare_pop_with_usb_dmac()
- usbhsf_dma_pop_done_with_usb_dmac()
So, if next transaction uses pio, the usbhsf_prepare_pop() can not
start the transaction because the "running" flag is 0.

commit 6cf439e0d37463e42784271179c8a308fd7493c6 upstream.
During _ffs_func_bind(), the received descriptors are evaluated
to prepare for binding with the gadget in order to allocate
endpoints and optionally set up OS descriptors. However, the
high- and super-speed descriptors are only parsed based on
whether the gadget_is_dualspeed() and gadget_is_superspeed()
calls are true, respectively.
This is a problem in case a userspace program always provides
all of the {full,high,super,OS} descriptors when configuring a
function. Then, for example if a gadget device is not capable
of SuperSpeed, the call to ffs_do_descs() for the SS descriptors
is skipped, resulting in an incorrect offset calculation for
the vla_ptr when moving on to the OS descriptors that follow.
This causes ffs_do_os_descs() to fail as it is now looking at
the SS descriptors' offset within the raw_descs buffer instead.
_ffs_func_bind() should evaluate the descriptors unconditionally,
so remove the checks for gadget speed.

commit 6180026341e852a250e1f97ebdcf71684a3c81b9 upstream.
There are 2 control endpoint structures for DWC3. However, the driver
only updates the OUT direction control endpoint structure during
ConnectDone event. DWC3 driver needs to update the endpoint max packet
size for control IN endpoint as well. If the max packet size is not
properly set, then the driver will incorrectly calculate the data
transfer size and fail to send ZLP for HS/FS 3-stage control read
transfer.
The fix is simply to update the max packet size for the ep0 IN direction
during ConnectDone event.

commit 46408ea558df13b110e0866b99624384a33bdeba upstream.
There is a race condition between finish_unlinks->finish_urb() function
and usb_kill_urb() in ohci controller case. The finish_urb calls
spin_unlock(&ohci->lock) before usb_hcd_giveback_urb() function call,
then if during this time, usb_kill_urb is called for another endpoint,
then new ed will be added to ed_rm_list at beginning for unlink, and
ed_rm_list will point to newly added.
When finish_urb() is completed in finish_unlinks() and ed->td_list
becomes empty as in below code (in finish_unlinks() function):
if (list_empty(&ed->td_list)) {
*last = ed->ed_next;
ed->ed_next = NULL;
} else if (ohci->rh_state == OHCI_RH_RUNNING) {
*last = ed->ed_next;
ed->ed_next = NULL;
ed_schedule(ohci, ed);
}
The *last = ed->ed_next will make ed_rm_list to point to ed->ed_next
and previously added ed by usb_kill_urb will be left unreferenced by
ed_rm_list. This causes usb_kill_urb() hang forever waiting for
finish_unlink to remove added ed from ed_rm_list.
The main reason for hang in this race condtion is addition and removal
of ed from ed_rm_list in the beginning during usb_kill_urb and later
last* is modified in finish_unlinks().
As suggested by Alan Stern, the solution for proper handling of
ohci->ed_rm_list is to remove ed from the ed_rm_list before finishing
any URBs. Then at the end, we can add ed back to the list if necessary.
This properly handle the updated ohci->ed_rm_list in usb_kill_urb().

commit a262e87ff354f12447bb6268bd63edf7ba1c20e0 upstream.
For historic reasons, the tegra platform selects USB_ULPI from architecture
code, but that hasn't really made sense for a long time, as the only
user of that code is the Tegra EHCI driver that has its own Kconfig
symbol.
This removes the 'select' statements from mach-tegra and drivers/soc/tegra
and adds them with the device driver that actually needs them.

On linux-4.4 and linux-4.9 we get a warning about an array that is
never initialized when CONFIG_REGULATOR is disabled:
drivers/usb/phy/phy-msm-usb.c: In function 'msm_otg_probe':
drivers/usb/phy/phy-msm-usb.c:1911:14: error: 'regs[0].consumer' may be used uninitialized in this function [-Werror=maybe-uninitialized]

drivers/usb/phy/phy-msm-usb.c:1912:14: error: 'regs[1].consumer' may be used uninitialized in this function [-Werror=maybe-uninitialized]
motg->v3p3 = regs[1].consumer;

drivers/usb/phy/phy-msm-usb.c:1913:14: error: 'regs[2].consumer' may be used uninitialized in this function [-Werror=maybe-uninitialized]
motg->v1p8 = regs[2].consumer;
This adds a Kconfig dependency for it. In newer kernels, the driver no
longer exists, so this is only needed for stable kernels.

commit 62a6abdd427b5fc4d8aad5dac874904e96273e6d upstream.
When dma_addr_t is 64-bit, we get a warning about an invalid cast
in the call to ux500_dma_is_compatible() from ux500_dma_channel_program():
drivers/usb/musb/ux500_dma.c: In function 'ux500_dma_channel_program':
drivers/usb/musb/ux500_dma.c:210:51: error: cast to pointer from integer of different size [-Werror=int-to-pointer-cast]
if (!ux500_dma_is_compatible(channel, packet_sz, (void *)dma_addr, len))
The problem is that ux500_dma_is_compatible() is called from the
main musb driver on the virtual address, but here we pass in a
DMA address, so the types are fundamentally different but it works
because the function only checks the alignment of the buffer and
that is the same.
We could work around this by adding another cast, but I have checked
that the buffer we get passed here is already checked before it
gets mapped, so the second check seems completely unnecessary
and removing it must be the cleanest solution.

Commit 76e0da34c7 ("usb-gadget/uvc: use per-attribute show and store
methods") caused a stringification of an undefined macro argument "aname",
so three UVC parameters (streaming_interval, streaming_maxpacket and
streaming_maxburst) were named "aname".
Add the definition of "aname" to the main macro and name the filenames as
originaly intended.

Commit 3eee23c3ec14 ("usbip: prevent vhci_hcd driver from leaking a
socket pointer address") backported the following commit from mailine.
However, backport error caused the tcp_socket address to still leak.
commit 2f2d0088eb93 ("usbip: prevent vhci_hcd driver from leaking a
socket pointer address")
When a client has a USB device attached over IP, the vhci_hcd driver is
locally leaking a socket pointer address via the
/sys/devices/platform/vhci_hcd/status file (world-readable) and in debug
output when "usbip --debug port" is run.
Fix it to not leak. The socket pointer address is not used at the moment
and it was made visible as a convenient way to find IP address from
socket pointer address by looking up /proc/net/{tcp,tcp6}.
As this opens a security hole, the fix replaces socket pointer address
with sockfd.

Upstream commit 1c9de5bf4286 ("usbip: vhci-hcd: Add USB3 SuperSpeed
support")
vhci_hcd clears all the bits port_status bits instead of clearing
just the USB_PORT_STAT_POWER bit when it handles ClearPortFeature:
USB_PORT_FEAT_POWER. This causes vhci_hcd attach to fail in a bad
state, leaving device unusable by the client. The device is still
attached and however client can't use it.
The problem was fixed as part of larger change to add USB3 Super Speed
support.
This patch isolates the one line fix to clear the USB_PORT_STAT_POWER
from the original patch.

commit cbeef22fd611c4f47c494b821b2b105b8af970bb upstream.
Quoting Hans:
If we return 1 from our post_reset handler, then our disconnect handler
will be called immediately afterwards. Since pre_reset blocks all scsi
requests our disconnect handler will then hang in the scsi_remove_host
call.
This is esp. bad because our disconnect handler hanging for ever also
stops the USB subsys from enumerating any new USB devices, causes commands
like lsusb to hang, etc.
In practice this happens when unplugging some uas devices because the hub
code may see the device as needing a warm-reset and calls usb_reset_device
before seeing the disconnect. In this case uas_configure_endpoints fails
with -ENODEV. We do not want to print an error for this, so this commit
also silences the shost_printk for -ENODEV.
ENDQUOTE
However, if we do that we better drop any unconditional execution
and report to the SCSI subsystem that we have undergone a reset
but we are not operational now.

commit ce5bf9a50daf2d9078b505aca1cea22e88ecb94a upstream.
Upon usb composition switch there is possibility of ep0 file
release happening after gadget driver bind. In case of composition
switch from adb to a non-adb composition gadget will never gets
bound again resulting into failure of usb device enumeration. Fix
this issue by checking FFS_FL_BOUND flag and avoid extra
gadget driver unbind if it is already done as part of composition
switch.
This fixes adb reconnection error reported on Android running
v4.4 and above kernel versions. Verified on Hikey running vanilla
v4.15-rc7 + few out of tree Mali patches.

commit c7b8f77872c73f69a16528a9eb87afefcccdc18b upstream.
According to drivers/usb/serial/io_edgeport.c, the driver may sleep
under a spinlock.
The function call path is:
edge_bulk_in_callback (acquire the spinlock)
process_rcvd_data
process_rcvd_status
change_port_settings
send_iosp_ext_cmd
write_cmd_usb
usb_kill_urb --> may sleep
To fix it, the redundant usb_kill_urb() is removed from the error path
after usb_submit_urb() fails.
This possible bug is found by my static analysis tool (DSAC) and checked
by my code review.

commit f0386c083c2ce85284dc0b419d7b89c8e567c09f upstream.
When disconnected sometimes the cdc-acm driver logs errors like these:
[20278.039417] cdc_acm 2-2:2.1: urb 9 failed submission with -19
[20278.042924] cdc_acm 2-2:2.1: urb 10 failed submission with -19

[20278.060418] cdc_acm 2-2:2.1: urb 15 failed submission with -19
